Skilled Nursing
Our Skilled Nurses at Sierra Hospice Care take charge of the Coordination of Hospice Care for clients. We also provide companionship and compassionate care in the areas of:
- Pain and Symptom Control
- Therapy Services (as needed)
- Diet Monitoring
- Assistance with Medication Monitoring and Administration
- Access to Hospice Care Equipment
If you do not have a Physician Prescribed Hospice Care Plan yet, please call 213-380-1100. Our team will coordinate with your doctor in the development and the implementation of the care plan.
We will also help in determining if hospice care is your best option given your health situation and living environment.
