Sierra Hospice Care is one of the participating care coordination agencies (CCA) to deliver the ALW Program to our client.
The services of a CCA include identifying, organizing, coordinating, and monitoring services needed by a participant. Care Coordinators conduct assessments to assist in determining eligibility, develop Individualized Service Plans (ISPs), arrange for services, maintain contact with participants, and monitor service delivery monthly.
Assisted Living Waiver Program (ALW)
The Assisted Living Waiver Program (ALW) is a comprehensive program designed to facilitate a secure, seamless transition for Medi-Cal-eligible seniors and persons with disabilities from a nursing facility to a community-like setting in a Residential Care Facility (RCF), an Adult Residential Care Facility (ARF), or public subsidized housing.
The ALW program not only aids individuals in transitioning from nursing facilities but also serves eligible seniors and persons with disabilities already residing in the community who are at risk of being institutionalized. The ALW services can be used to create a program tailored to safely meet their care needs, allowing them to continue living in an RCF, ARF, or public subsidized housing.
Eligibility Criteria
Eligible candidates for the ALW program are those enrolled in Medi-Cal and whose care level has been established by a medical professional’s assessment. Please note that individuals with Medi-Cal benefits, which include a “share of cost”, may not be enrolled in the ALW program.
Pre-Screening Process
The pre-screening process is the initial step toward the ALW program. Here, applicants can complete an intake application for pre-qualification. Our office will carefully review your application and inform you of your enrollment status.
Enrollment Process
Once your pre-screening application is accepted, we will move forward with the enrollment process. Our office will schedule an in-person assessment with a registered nurse (RN) to determine the required level of care. This process is an integral part of the enrollment as it allows us to understand your specific care needs. We will then assist you in gathering the additional required documents to complete the admission approval process.
