Admission to Hospice Care
Please call our office for assistance and a nurse will visit you or your loved one at home or in the care facility (skilled nursing facility, hospital or other health care center). We will provide you with information about the hospice care program and will also answer any concerns you may have about what to expect with hospice care from Sierra Hospice Care.
Once you and your family decide to receive hospice care, our nurse will make schedule arrangements and will also work with your physician in the development of a specialized care plan to address your palliative care needs.
