POLICIES & PROCEDURES for PATIENT COMMENTS, QUESTIONS, or COMPLAINTS

If you have any questions or comments or are dissatisfied with our services, you may call or write to the Administrator or Director of Patient Care Services.
Services will not be diminished or discontinued if you or a family member registers a complaint. Your complaint will be investigated within seventy-two (72) hours of receipt. We will advise you of our findings and the action taken to resolve your complaint.
You may also contact the State Home Health Agency Toll-Free Hotline, listed below 24 hours a day, seven days a week. The purpose of this hotline is to receive complaints or questions about local home agencies.
This hotline also receives complaints concerning the implementation of advance directive requirements:
